📸 Photo/Video Usage: Photographs and video recordings will be taken during program activities for documentation and promotional purposes. Media will only be used in appropriate contexts (e.g., newsletters, flyers, social media, or funder reports). We will never publish identifying information without your written permission.
⚠️ Participation Waiver: The parent/guardian understands that participation in the Bob-O-Links Junior Golf Club involves physical activity, and acknowledges and accepts the inherent risks. I agree not to hold the organizers, staff, facilities, or program partners liable for any injuries or accidents that may occur during program activities.
🚗 Emergency Transportation: In the event of a medical emergency where the parent/guardian cannot be reached, the program staff are authorized to secure emergency care and transport my child to the nearest medical facility, if deemed necessary.
📘 Code of Conduct: All participants are expected to behave respectfully, follow safety rules, and listen to coaches and staff. Repeated violations of the Code of Conduct will result in removal from the program.

I/WE AGREE TO ENROLL MY/OUR CHILD IN THE BOB-O-LINKS JUNIOR GOLF CLUB PROGRAM AND WILL PROVIDE THE NECESSARY TRANSPORTATION WHEN NEEDED. I UNDERSTAND THAT MY CHILD IS TO ARRIVE AT EVENTS ON TIME AND WILL BE PICKED-UP TIMELY.
AS THE PARENT/GUARDIAN OF THE NAMED YOUTH, I UNDERSTAND THAT ONLY THOSE WHO MAINTAIN A HIGH DEGREE OF REGARD AND RESPECT FOR PEERS, TEAMMATES, INSTRUCTORS, GUESTS, AND VISITORS WILL BE PERMITTED TO CONTINUE MEMBERSHIP IN THIS PROGRAM.
By e-signing participants in the Bob-O-Links Junior Golf Club, you are waiving and releasing all claims for injuries/liability, which may occur during all program events and activities.

I/We grant permission and consent to Bob-O-Links Junior Golf Club (“BOL”) and/or the Chicago Women’s Golf Club (“CWGC”) the use of my child’s/guardian’s photograph(s) as identified below for presentation under any legal condition, including but not limited to: photography, videography, publicity, copyright purposes, illustration, advertising, books, brochures, digital content, DVD’S, CD’S, non-fungible token (NFT), social media and web content.
The BOL and CWGC respect your privacy and are committed to protecting your personal data and information. The images we collect may be published online on our website, intranet, social media profiles, advertising, or in print. Your image may also be used for teaching, fundraising or awareness campaigns, and for training and development. We will only use your other information to identify you to ensure that we comply with your individual rights.
By e-signing participants in Bob-O-Links, you are granting permission and consent to the above release statement for photography, videography, etc.
PARTICIPATION SURVEYS
Please complete the Bob-O-Links Junior Golf Club PRE-PROGRAM SURVEY prior to the first class in April and the POST-PROGRAM SURVEY at the completion of the program in August. We want to serve our junior golfers and their families effectively and completely. Your feedback is greatly appreciated.
