📸 Photo/Video Usage: Photographs and video recordings will be taken during program activities for documentation and promotional purposes. Media will only be used in appropriate contexts (e.g., newsletters, flyers, social media, or funder reports). We will never publish identifying information without your written permission.

⚠️ Participation Waiver: The parent/guardian understands that participation in the Bob-O-Links Junior Golf Club involves physical activity, and acknowledges and accepts the inherent risks. I agree not to hold the organizers, staff, facilities, or program partners liable for any injuries or accidents that may occur during program activities.

🚗 Emergency Transportation: In the event of a medical emergency where the parent/guardian cannot be reached, the program staff are authorized to secure emergency care and transport my child to the nearest medical facility, if deemed necessary.

📘 Code of Conduct: All participants are expected to behave respectfully, follow safety rules, and listen to coaches and staff. Repeated violations of the Code of Conduct will result in removal from the program.
